HIFU vs Thread Lift — Which Non-Surgical Facelift Is Better?

Sagging skin around the jaw, dropped brows, a neck that's started to look loose. These changes build slowly and then one day bother you a lot. Surgery fixes them decisively, but it's a significant step that most people would rather avoid.

HIFU and PDO thread lifts both offer real, visible lifting without going under the knife. They do it in completely different ways though — and they suit different patients.

What HIFU Does

HIFU (High-Intensity Focused Ultrasound) delivers ultrasound energy to precise depths inside the skin without breaking the surface. No needles, no cuts. The energy creates tiny heat points at specific tissue layers — reaching 4.5mm depth, which is the SMAS layer, the same layer that surgeons tighten during a traditional facelift.

At that depth, tissue contracts immediately. Then over the following 3 to 6 months, new collagen builds progressively, continuing to tighten long after the treatment day. Patients often notice the results keep improving for months after they'd almost forgotten about it. One session per year is typically enough to maintain the effect.

What a Thread Lift Does

A PDO thread lift takes a mechanical approach: fine polydioxanone threads — a material with decades of safe surgical use — are inserted under the skin through fine needles. The barbs on the threads physically grip and reposition sagging tissue. You can see and feel the lift on the day of treatment.

The threads dissolve over 6 to 8 months, but the collagen stimulation they trigger continues well after that. Results typically hold for 18 to 24 months. The downside: 3 to 5 days of mild swelling and some tenderness. Not painful, but enough that you'd want to avoid an event that week.

Which One Actually Gives Better Results?

Depends what you mean by “better.” If you need to see a result quickly — a wedding in a few weeks, an upcoming event, or you simply want immediate reassurance — threads win. Patients consistently describe that feeling of looking in the mirror after thread placement and seeing where their face sat a few years ago.

For deeper, more structural change, HIFU reaches further into the tissue. It's particularly good for the neck and lower jaw, areas where threads can be trickier to place and where the SMAS-level collagen remodelling HIFU triggers is closest to what a surgical facelift achieves.

Combining both is actually our most common recommendation: threads first for the immediate lift, HIFU 4 to 6 weeks later to deepen and extend the result. Many patients add Botox and fillers to that as well — which rounds out the upper face and restores volume that lifting alone won't address.

Who Should Choose HIFU

Zero downtime is the main appeal. HIFU is also a strong choice for patients in their mid-30s to early 50s looking for gradual, preventive tightening before significant laxity develops, and for anyone whose primary concern is the neck and lower jawline. Patients who are wary of needles often prefer it too.

Who Should Choose a Thread Lift

Choose threads if you want to see something happen quickly, you have moderate sagging that needs real mechanical repositioning, or you have specific problem areas like heavy jowls or a noticeably dropped brow. Results lasting up to 24 months is also a strong point — HIFU typically needs a top-up session at around 12 to 18 months.
