Teeth whitening is one of those treatments where the gap between what people expect and what they actually get can be massive — depending entirely on whether they go to a clinic or buy a box from a pharmacy. A whitening toothpaste and a professional in-clinic session are not even remotely in the same category.
Why Teeth Go Yellow
Two different types of staining, two different solutions.
Surface stains sit on the outer enamel. Tea, coffee, cola, tobacco — these cause external discolouration that a good professional scale and polish removes. The trickier type is intrinsic staining, which lives inside the tooth structure itself. It happens with age, after certain medications like tetracycline, through fluorosis, or simply genetics. No amount of brushing reaches it. You need a bleaching agent that penetrates the enamel from within.
How Professional Whitening Works
Professional whitening uses hydrogen peroxide or carbamide peroxide gel at clinical concentrations — far higher than anything available over the counter. The gel penetrates the enamel and oxidises the colour compounds trapped inside. The result is a lighter tooth from within, not a cleaner surface.
At our clinic, we always do a scale and polish first, before any whitening gel goes on. This matters more than most people realise. A bleaching agent works best on clean enamel with no tartar or biofilm sitting in the way. Skipping this step is probably why a lot of people have underwhelming results elsewhere.
The gel sits on the teeth for 45 to 60 minutes in clinic. Most patients come out 3 to 8 shades lighter in a single appointment.
Professional vs At-Home: The Actual Difference
| Factor | Professional (Clinic) | At-Home Kits |
|---|---|---|
| Peroxide concentration | Clinical grade, significantly higher | Low, regulated for consumer safety |
| Reaches intrinsic stains | Yes | Barely |
| Shade improvement | 3 to 8 shades, one session | 1 to 2 shades over 2 to 6 weeks |
| How long results last | 12 to 24 months | 3 to 6 months |
| Gum protection | Professionally applied barrier | User-applied, irritation risk if imprecise |
What Whitening Won't Do
Whitening only works on natural tooth enamel. Crowns, veneers, composite fillings — none of these change colour with bleaching. If you have visible restorations on your front teeth, whitening the natural teeth around them creates a mismatch. Worth knowing before you book.
In that case, the options are replacing those restorations after whitening to match the new shade, or skipping whitening entirely and looking at porcelain veneers. Veneers don't stain at all and maintain their shade for 10 to 15 years, which makes them a better long-term choice for anyone who wants a permanently white smile rather than one that needs topping up.
Is It Safe?
Professional whitening is very safe when done properly. The most common side effect is temporary sensitivity, which settles within 24 to 48 hours for most patients. We assess enamel condition before every whitening appointment — anyone with thin enamel or existing sensitivity gets a sensitivity toothpaste protocol for a week beforehand, which genuinely helps.
The thing to avoid: repeatedly using at-home kits without any professional assessment. Overuse can genuinely damage enamel over time. In-clinic whitening done once or twice a year is safe indefinitely for the vast majority of patients.
Who It's For
Whitening works well for adults with healthy teeth and gums who have yellow or brownish staining from diet, age, or tobacco. It's also popular before weddings and events — results are at their best in the first few weeks.
Whitening isn't recommended during pregnancy, for patients with untreated cavities or active gum disease, or for severe tetracycline staining. That last one is important: tetracycline staining goes very deep into the dentine and responds poorly to bleaching. Cosmetic dentistry options like veneers are usually the better answer for that specific case.
How Long Do Results Last?
12 to 24 months for most patients, but lifestyle makes a real difference. Heavy tea and coffee consumption, smoking, and skipping regular cleanings accelerate staining noticeably. Using a straw for coloured drinks, rinsing after meals, and keeping up with 6-monthly scale and polish appointments all extend results significantly.
