Wisdom Teeth Removal — When Is It Necessary & What to Expect

Wisdom teeth often cause discomfort, crowding, or infection. Here is what you need to know about when removal is required and what recovery looks like.

Wisdom teeth — officially known as third molars — are the last set of permanent teeth to emerge, typically appearing between ages 17 and 25. For some people, wisdom teeth erupt smoothly with no issues. But for many patients in Rawalpindi and Islamabad, there simply isn't enough space in the jaw for them to grow correctly.

Left unmanaged, impacted or partially erupted wisdom teeth can lead to recurring pain, jaw infections, and damage to neighboring molars. Here is a clear guide on how to tell if yours need to come out and what modern, comfortable extraction involves.

5 Clear Signs Your Wisdom Teeth Need Removal

Not all wisdom teeth need to be extracted immediately. However, dental surgeons generally recommend oral surgery extractions when any of the following symptoms or conditions arise:

  • Pericoronitis (Gum Flap Infection): When a tooth partially breaks through the gum, food particles and bacteria get trapped under the gum flap, leading to foul taste, swelling, and throbbing pain.
  • Impacted Growth: The tooth is growing horizontally or at an angle toward the adjacent molar, pushing against healthy teeth.
  • Crowding & Alignment Shift: Pressure from erupting third molars forces front teeth to overlap or misalign.
  • Cyst Formation: A fluid-filled sac forms around the sac of the wisdom tooth, which can damage jawbone tissue and neighboring roots.
  • Uncleanable Cavities: Wisdom teeth situated far back in the mouth are difficult to brush and floss, making them prone to severe decay.

Simple vs. Surgical Wisdom Tooth Extraction

Depending on the tooth's position, extractions fall into two categories:

Simple Extraction: If the wisdom tooth has fully erupted above the gumline and has straight roots, the dental surgeon can gently loosen and remove it under standard local anaesthesia in a quick 15-to-20-minute procedure.

Surgical Extraction: If the tooth is impacted beneath the gum or bone, a minor surgical approach is used. A small incision is made in the gum tissue, and the tooth may be divided into smaller sections for gentle, painless removal with minimal disruption to surrounding tissue.

Is Wisdom Tooth Removal Painful?

During the procedure itself: No. Modern local anaesthetics ensure the area is completely numb before any work begins. You will feel pressure, but no sharp pain. For anxious patients, mild sedation options can make the experience entirely stress-free.

After the procedure, mild swelling and soreness are normal for 3 to 5 days, but these are easily controlled with prescribed pain medication and cold compresses.

Step-by-Step Recovery Timeline

  1. First 24 Hours: Rest with your head elevated. Keep gentle pressure on the gauze pad for 30–45 minutes post-surgery to stop bleeding. Avoid rinsing, spitting, or drinking through a straw.
  2. Days 2 to 3: Swelling peaks and starts to subside. Stick to soft, cool foods like yogurt, soups, and smoothies. Gently rinse with warm salt water after meals.
  3. Days 4 to 7: Normal eating resumes gradually. Stitches (if non-resorbable) are evaluated or removed, and jaw stiffness resolves.

How to Prevent Dry Socket

A dry socket occurs when the blood clot that forms in the extraction socket is dislodged prematurely, exposing the underlying bone. To prevent this:

  • Do NOT smoke or use vape products for at least 72 hours post-extraction.
  • Do NOT use a drinking straw, as the suction can pull out the clot.
  • Do NOT perform vigorous mouth rinsing during the first 24 hours.
